How to register

Registering with Peel Hall Medical Practice

New patients living in our catchment area are welcome to join. Follow these steps to register with us:

1. Open the online registration form

2. Fill in the form and click the ‘Send’ button to mail your form to us.

3. Visit the surgery with proof of address, a photo ID and signature. If you have any of the following documents, please bring them with you (but not essential):

  • Photo ID: passport/driving licence
  • Proof of address in catchment area: current utility bill
  • For children under 5 years of age: red baby book
  • For children up to 18 years if no photo ID available: birth certificate

4. Book an appointment for a new patient health check to complete your registration.

5. The patient when joining can stipulate whether they wish to have a designated mal or female or have a chaperone present at their consultations.

6. Having your NHS helps greatly in enrolling new patients, you find you existing NHS number by clicking here.

Please note that children aged 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the form on their behalf.

Change of details (only to be completed by existing patients)

It is important that any current patient informs us of any changes in their name, address, phone number(s) or email address so that we can contact them when required.

Please click here and complete the admin form if you are an existing patient and your contact details have changed, such as mobile number or address where you live.

If you move outside the practice area, you may be asked to find another practice.

Accountable Named GP

Every GP practice is required by the Government under the terms of the latest GP contract to allocate all patients an accountable named GP.

We have allocated every patient a GP who will be responsible for their overall care and a member of the reception team will be able to confirm the name of your accountable GP.

Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.

If a patient expresses a preference as to which GP they are assigned to, the Practice will make reasonable efforts to accommodate their request.
