If you have a health concern, you can contact us using the online form, where you can fill in a form that will be passed to a doctor or nurse at the practice. This is an easy, convenient way to contact us and it can save you time by avoiding busy telephone lines. You can fill in an online form during morning surgery hours (which are on our home page).
- Open the online form there is no need to download an app or create an account.
- Select either admin or medical request, see image below..
- Confirm this is a non urgent request.
- Write the request to the practice, giving as much information as possible.
- Tell us how you wish to be contacted.
- Enter your details, please check your details are correct as incorrect or missing details will lead to a delay in dealing with your request.
- Finally, press submit.
Once you have sent the form, your query will be acknowledged, it can take up to two working days to process your form. We will then call you to arrange a telephone appointment with a doctor or nurse.

If you need urgent medical help at any time, please consider the following:
- Call 999 immediately if you or someone else is having a heart attack, stroke or has suffered a major trauma, such as a road traffic accident, a fall from height or a serious head injury.
- Please contact NHS 111 for any urgent but not life-threatening health problem. You can either call 111 or visit 111.nhs.uk 24 hours a day, 365 days a year.
- Contact the out-of-hours service by calling 0161 375 1000, where your call will be re-directed for medical assistance outside of normal surgery hours
